2 · Leagues
Only if you're playing competitively. For each league, say whether you're playing and, for South & West Middlesex, which division.
3 · British Clubs Leagues
Junior and cadet players only. Entry is charged per league, so entering both costs twice.
4 · Actonians membership
Everyone playing at the club also joins Actonians Sports Association. The club collects this alongside your Oaks fee.

How the club uses your details
The Oaks Club ("the club") is responsible for this information. We use it to run your membership and take payment. Our lawful basis is your membership with the club and our legitimate interest in running it; any medical information is special-category data that we hold only with your explicit consent (the tick box above).
Your details are stored in Google Sheets, and card payments are handled by Stripe — both act as our providers under contract, and your card number is handled only by Stripe, never by the club. Data may be processed outside the UK under approved safeguards.
We keep your details only as long as needed for your membership and delete them within [CONFIRM: e.g. 12 months] of it ending — except payment records, kept [CONFIRM: e.g. 6 years] for accounts. To see, correct or delete your data, or to object to its use, contact [CONFIRM: club contact]. You can also complain to the ICO at ico.org.uk.
